Bridge Oak is seeking an experienced Care Plan Manager to drive excellence in care planning across multiple homes in West Yorkshire. You will partner with Home Managers and clinical teams to ensure person-centred, accurate and outcome-focused plans.
You will provide oversight, guidance and assurance to maintain high standards, audit processes, coach teams and share best practices across services. Salary discussed at interview.
